Interventions projects progress
Design and implement work plans and obtain approval from the supervisor; collaborates with multidisciplinary healthcare teams to ensure coordinated
patient care; assess, diagnose and treat patients with physical impairments or functional limitations to restore movement, reduce pain, and improve
independence and quality of life; educate patients and caregivers on self-management, prevention, and home exercise programs while documenting
progress and adjusting treatment based on patient response.
Timeliness and quality of reports
Collaborate with head of CRC and provincial coordinator to ensure timely and accurate reporting; maintain filing systems and accurate clinical records and
reports.
Parts of programs and project proposals
Assist data collection and analysis, and identify good practices under the project.
Planning, monitoring and evaluation
Continuously monitor patients’ functional progress by regularly assessing mobility, strength, pain levels, and activity limitations against the established
rehabilitation goals; monitor adherence to home exercise programs and identifies any complications or barriers that may affect recovery.
Result communication and open data
Maintaining accurate and timely documentation of patient progress, monitor the effectiveness of rehabilitation interventions, and report significant changes
or concerns to the multidisciplinary team.
Monitoring of partners
Monitor and contribute to case reviews, ensuring continuity of care, and supporting quality improvement by identifying gaps in service delivery and
recommending corrective actions.
Capacity of partners regarding accountability
participate in capacity building and professional development activities as required.
Integrity & Compliance
Adheres to Cordaid’s Code of Conduct and integrity policies, ensuring ethical and professional engagement with beneficiaries.
Reports any concerns as per the established procedures.
