Job Descriptions:

The Community Health Supervisor (CHS) oversees community-based health activities to ensure effective delivery of health promotion, disease prevention, and outreach services. The role focuses on supervising and strengthening the capacity of Community Health Workers, monitoring program implementation, and ensuring timely, accurate data collection and reporting. The CHS collaborates with health facilities and key stakeholders to improve service coordination and expand community access to essential health and nutrition services, contributing to improved health outcomes.

• Community health supervisor is overall responsible for planning, executing and reporting all CBHC activities as per project scope of work and Ministry of Public Health policies and maintain close working coordination of local communities with health facility.

• Assist HF in-charge for the establishment of HF and health post level community health shuras with active participation and involvement of local community elders.

• Conduct regular CHWs health posts visits and attend HF and HP level local coordination and monthly health committee meetings.

• Organize and facilitate CHWs trainings and conduct supportive supervision from all HP within HF catchment area with main focus on CHWs on the job training, receiving feedback from local communities/clients and reporting to HF.

• Prepare schedule for monthly health Shura meetings and ensure full and active participation of community elders.

• Support Health facility to organize services in terms of clients/patients flow control/screening.

• Maintain regular communication and establish close working coordination between community and health facility.

• Responsible to support community in finding and solving health problems.

• Assist HF staff in providing plans of community health programs in the health facility and the catchments areas.

• Conduct community awareness sessions as per Health Facility plan and IEC materials given by Health Facility In-charge/doctor on regular basis.

  1. Conduct regular health education sessions according to the health facility health education plan and organize public awareness sessions in target communities and HP level with active involvement of CHWs.
  2. Regular reporting to health facility regarding all CBHC activities and CHWs monthly performance.
  3. To use and report Health committee information to monitor the health status and deliver the report to head of health facility
  4. Use health committee data to monitor community health status

Compile and share reports with the Health Facility In-Charge to support decision-making

 

• Reports to health facility In-Charge and field supervisor

• Coordinate with members of the basic health centre (BHC), and the Provision of Essential Life-Saving Healthcare Services to catchment are population as per CBHC policy and project scope of work.            

1. Key Results Expected:

1- Coordination Established between Health Facility and community in the surrounding areas of the Health Facility

2- Local community awareness level improved regarding the health care services available in the health facility.

3-Community level referral of increased to Health Facility.

4-Health promoted in the target areas of the health facility

Job Requirements:

Education: 

•Graduated from 12 grade at least, and higher education level will be preferred especially medical institute.

Work Experience:

• The Candidate must have at least one-year previous experience in the relevant field or similar working context.

Languages Skills:

• Fluency in spoken and written Dari and Pashtu is a requirement. English Language skills is a plus point.

Previous working experience (at least one year in the communities mainly involved in health education, awareness, community mobilization and other social works).

Level 1: What has to be done and how to do it are clearly defined, and the incumbent will face identical or similar problems on a regular basis:

The Community Health Supervisor manages CBHC activities, supervises CHWs, coordinates with communities, and ensures reporting in line with MoPH policies
